Registered Nurses (RNs) & Licensed Practical Nurses (LPNs)
Position: Continuous Care Nurse
At Home Health Solutions Group, we are committed to delivering compassionate, high-quality care that allows patients to remain comfortable in their own homes. We are currently seeking dedicated Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to join our growing Continuous Care team.

What You'll Do
- Provide one-on-one skilled nursing care in the patient's home.
- Monitor and assess patient conditions.
- Administer medications and treatments as ordered.
- Collaborate with physicians and the interdisciplinary care team.
- Educate patients and families on disease management and care plans.
- Accurately document patient care in accordance with agency policies.

Qualifications
- Current RN or LPN license for at least 1 full year.
- Current CPR/BLS certification.
- Home health or hospice experience preferred but not required.
- Strong clinical, communication, and critical thinking skills.
- Compassionate, dependable, and committed to providing exceptional patient care.
